• Full Preferred Name
• Preferred Pronouns
• Date of Birth
• Procedure(s) you are looking to have covered
• Scan of your insurance card (both front and back)
• Therapist/Doctor letters of support for your surgical procedures. These letters should state clearly that you have “gender dysphoria” related to the area you wish to have surgery on, and that you “meet WPATH criteria” for surgery.
